PLEASE ENSURE THAT WE RECEIVE YOUR COMPLETED QUESTIONNAIRE BY 30TH SEPTEMBER 2012. If we do not receive your completed form by this date then we cannot guarantee that the filing deadline will be met.

In particular, please ensure we have all information by 31st October or we will have to levy a fee of £175 + VAT.
